Volunteer Waiver

Printed copies will not be accepted. Electronic waivers are the only acceptable format.
If you are under the age of 18, a parent or guardian must complete the waiver with the minor’s name in the
first and last name fields within the waiver and parent/guardians’ signature at the bottom.
